Collaborates with health care providers to ensure eligible children/youth receive quality health care. Assists families with finding a primary care physician and dentist.CHDP prentive health assessments includes: Complete Health and Developmental History Behavioral Screening Lab tests for anemia, lead, TB (Tuberculosis), and other problems as needed Unclothed physical exam by a physician (heat to toe) Immunizations An oral examination and Denti-Cal services A nutritional assessment Health education for patients and parents WIC referrals for children through age 5
